The Preparation for Adulthood (PFA) Service is looking for a dedicated Assertive Outreach Support Worker to join our citywide team, helping young people through the important transition from Children's Social Care to Adult Social Care.
About the Role:
As an Assertive Outreach Support Worker, you will:
- Build relationships with young people at risk or in need of support, helping them engage with appropriate services.
- Carry out strengths-based, holistic assessments to ensure individual needs, goals, and aspirations are met.
- Support young people to develop independent living skills and connect with their communities.
- Work closely with partners including health providers, education staff, police, and voluntary organisations to deliver joined-up, person-centred support.
- Promote the service to partner agencies and advocate for the rights and needs of vulnerable young people across Manchester.
- Strong communication, planning, and analytical skills.
- A non-judgemental, empathetic approach to working with young people.
- Ability to manage risk, assess needs, and co-produce support plans.
- Commitment to equality, diversity, and inclusion in all areas of work.
- Willingness to undertake NVQ Level 4 in Health and Social Care (if not already held).
- Enhanced DBS check required.
