Tameside Children’s Services are recruiting Transition Support Workers to be part of the Staying Close project — an innovative service designed to support young people aged 16–21 who are leaving residential care. This is a unique opportunity to help shape the future of care leavers, providing them with the practical, emotional, and social tools needed for a successful transition to independent living.

About the Role:
As a Transition Support Worker, you’ll build strong, meaningful relationships with young people who have recently left care, helping them navigate adulthood with confidence. Your support will cover everything from employment and education to daily living skills like budgeting, cooking, and managing a tenancy. You’ll be a consistent figure in their lives, helping them access opportunities and overcome challenges.

What We're Looking For:
? Level 3 qualification in childcare or related subject
? Experience supporting young people aged 16–25 with complex needs or vulnerabilities
? A background in residential care or youth work
? Strong knowledge of independence-building skills: budgeting, shopping, cooking, bills, and housing support
? A passionate, positive, and person-centred approach
