Islington council is recruiting for Hospital social worker for the new rapid project team
fro this role you will
- Give Patients get a 7 day care package, Social worker required to do assessments to see if that needs extending, modifying or if they need to move to reablement service or complete a care act assessment and hand them off.
- work closely with the reablement team
- complete assessments in a timely manor
- report back to team manager and hand patients over
- rapid hopsital discharge
