Outreach Family Support Worker

Job Description


Liverpool Early Help is seeking a passionate and dedicated Outreach Family Support Worker to join a well established team. Their approach is strength-based, focused on empowering children and families by addressing emerging concerns and working together to improve long-term outcomes. As a lead professional, you will coordinate support with multi-agency partners, ensuring a whole-family approach to addressing needs and bringing about positive change for children and young people.

  • Monday to Friday 9 to 5
  • 35 Hours per week
  • Initial 3 month contract with the possibility of extension

Responsibilities:

  • Conduct early help assessments with families to identify needs and areas for support.
  • Develop and implement support plans tailored to the family's needs.
  • Chair Team Around the Family (TAF) meetings to review progress and ensure continued support.
  • Work directly with children and young people, gaining insights into their views and worries.
  • Collaborate with multi-agency partners to provide comprehensive support.
  • Use a whole-family approach to ensure holistic and effective interventions.
  • Ensure interventions positively impact children and young people's long-term outcomes.

Requirements:

  • Experience working with children, young people, and families, particularly in early help or social care settings.
  • Strong understanding of early help assessments, support planning, and multi-agency working.
  • Excellent communication and interpersonal skills to engage effectively with families, professionals, and young people.
  • Ability to chair meetings, review progress, and adapt plans to changing family needs.
  • A commitment to safeguarding and improving the well-being of children and young people.
