Responsibilities:
- Conducting mental health assessments to understand patients' needs, concerns, and risks.
- Developing personalized care plans with the patient, family, and multidisciplinary teams.
- Providing immediate support to patients in mental health crises, which could include prevention of hospital admission.
- Liaising with social workers, occupational therapists, and other health professionals to coordinate comprehensive care.
Qualifications and Requirements:
- Fully enhanced valid DBS
- Mental Health Nursing degree required.
- Community experience is required.

Rate Breakdown:
£27ph LTD | £24.29ph PAYE Inclusive
We offer a £250 bonus for starting a contract with SCS.
