Community Psychiatric Nurse (CPN) - Worcestershire and Herefordshire NHS

Job Type: Temporary Contract
Working Hours: Monday to Friday, 9:00 AM to 5:00 PM
Pay Rate: £27.00 per hour (Ltd, paid via umbrella company)

Key Responsibilities:

  • Conducting assessments for individuals requiring mental health support.
  • Acting as a Care Coordinator (CCO) for patients, ensuring a structured approach to care delivery.
  • Engaging in triage activities to prioritise and assess the needs of service users.
  • Providing care and support to individuals with a diagnosis of psychosis, utilising clinical expertise and knowledge to help manage and improve their mental health.

Requirements:

  • Experience working with people diagnosed with psychosis.
  • Strong knowledge of mental health assessments, triage processes, and care coordination.
  • Registered with the NMC (Nursing and Midwifery Council) and have valid qualifications as a Community Psychiatric Nurse.
  • Previous experience working in a similar role within the NHS is highly desirable.

Person Specification:

  • Registered Nurse (Mental Health): Must hold a current NMC registration.
  • Experience in Psychosis Care: Demonstrable experience working with individuals diagnosed with psychosis, preferably within a community mental health setting.
  • Care Coordination (CCO) Experience: Experience acting as a Care Coordinator, managing patient caseloads, developing care plans, and coordinating services for individuals with complex mental health needs.
  • Assessment & Triage Skills: Strong ability to conduct mental health assessments, manage risk, and provide triage support, prioritizing care based on the severity of the individual's condition.

  • Communication Skills: Excellent verbal and written communication skills, with the ability to engage and build rapport with service users, families, and multidisciplinary teams.
  • Organizational Skills: Effective time management, able to manage a busy caseload, prioritize tasks, and work within deadlines.
  • Teamwork: Strong collaborative approach, working effectively within multidisciplinary teams, and contributing positively to team dynamics.
  • Clinical Knowledge: Up-to-date knowledge of mental health legislation, including the Mental Health Act, and an understanding of the challenges faced by individuals with severe mental illnesses.

Other Requirements:

  • DBS Clearance: A valid, up-to-date Disclosure and Barring Service (DBS) check, ideally on the update service.
  • Driver's License: A full UK driving license is preferable, as the role may involve visiting clients in the community.
