- Occasional Weekend Working may be required.
- Weekdays: £26 per hour
- Saturdays and Nights: £30 per hour
- Sundays and Bank Holidays: £34 per hour
Role Overview We are seeking two experienced and dedicated Senior Mental Health Practitioners to join our East Suffolk Crisis Resolution Home Treatment Team based at Woodlands Ipswich Hospital. The successful candidates will be working with individuals in the community who are experiencing acute mental health crises, helping to prevent hospital admissions by providing treatment and support within the home.
You will play a vital role in assessing, supporting, and managing service users with mental health needs in crisis, assisting them to remain in their community and avoid hospitalisation. Your work will involve a combination of assessments, care management, medication oversight, and liaising with various stakeholders, including families, other mental health services, GPs, and carers.
Key Responsibilities
- Assessment & Care Needs: Conduct thorough assessments of service users to establish their current mental health needs and care requirements.
- Support & Liaison: Provide support to service users, families, and carers, acting as a primary point of contact for those in crisis.
- Medication Management: Manage and monitor medication as part of the treatment plan for service users.
- Collaborative Working: Liaise with other secondary mental health services, ensuring continuity of care and proper coordination of support.
- Signposting: Guide service users and carers towards appropriate services and resources as needed, particularly when stepping down from crisis care.
- Duty Role (Occasional): With appropriate training, act as a duty worker, coordinating appointments, joint visits with other services, and maintaining contact with service users and their families.
- Support & Supervision: Provide support, learning, and guidance to students within the team, contributing to their professional development.
- Liaison with GPs: Offer advice and guidance to GPs and other healthcare professionals, ensuring the integration of community-based care.
- Qualifications: A relevant qualification in Mental Health Nursing (e.g., RGN with a mental health qualification), registered Social Worker, Dip OT, Clinical Psychology, or equivalent.
- Experience: Proven experience in a community mental health setting, particularly in crisis resolution or home treatment.
- Knowledge & Skills: A strong understanding of mental health assessments, crisis intervention, medication management, and support planning.
- Personal Skills: Excellent communication and interpersonal skills, with the ability to build rapport with service users and work collaboratively with multi-disciplinary teams.
- Flexibility: Ability to work across a range of shift patterns, including occasional weekend work and nights.
- Location: Woodlands Ipswich Hospital is located in Ipswich, Suffolk. This role requires presence at the hospital and cannot be done remotely.
