Service Care Solutions has a new and exciting opportunity which has become available working as a Transitional support Worker with Tameside Council

Tameside Council are currently looking for someone who is happy to work 37 hours per week.

Key Responsibilities:

  • Establish face-to-face contact with young people and engage them with the service.
  • Conduct needs assessments and develop tailored plans to help young people achieve their aspirations.
  • Provide support for accessing education, training, employment, and community-based activities.
  • Liaise with external agencies to secure necessary support for young people, including housing, health services, and therapy.
  • Deliver group sessions and positive activities that promote personal growth and community involvement.
  • Uphold safeguarding principles and respond appropriately to any concerns.
  • Advocate for young people's voices, ensuring their views shape the services provided.
  • Maintain accurate records and manage caseloads effectively, adhering to departmental procedures.
  • Contribute to the ongoing development of the Staying Close Service.

About You:
We're looking for a compassionate, motivated individual who is passionate about supporting young people.

Essential Qualifications:

  • NVQ Level 3 in Care of Children and Young People, Adult Social Care, or equivalent.

Essential Skills and Experience:

  • Proven experience working with Cared-for Children and families.
  • Familiarity with Child Protection Procedures and relevant legislation for Care Leavers.
  • Strong IT, administrative, and organizational skills.
  • Ability to build meaningful relationships with young people and their families.
  • Knowledge of child development, attachment theory, and effective assessment techniques.
  • A commitment to anti-discriminatory and equal opportunity practices.

Desirable Skills and Experience:

  • Experience supporting families with substance abuse or domestic violence concerns.
  • Knowledge of parenting capacity assessments and evidence-based practices.
  • Understanding of care planning and court processes.
  • Full driving licence and access to a car for business use.
