Social Worker – South Community Adult Social Care Team

About the Role

An exciting opportunity has arisen for an experienced Social Worker to join the South Community Adult Social Care Team at Bristol City Council. This is a fast-paced and rewarding role where you will support adults in the community by completing assessments, reviews, and support planning under the Care Act 2014.
You will work in a multi-disciplinary environment, collaborating with health, housing, and voluntary organisations to deliver person-centred and strength-based support to Bristol’s diverse communities.

Key Responsibilities
  • Undertake Care Act assessments, carer assessments, and reviews, ensuring individuals receive the right level of support.
  • Develop support plans that promote independence, choice, and well-being.
  • Complete mental capacity assessments and best interest decisions where required.
  • Lead on safeguarding enquiries, working in line with safeguarding policies and procedures.
  • Work closely with health, housing, and voluntary sector partners to ensure effective multi-agency support.
  • Maintain accurate and timely records in line with statutory requirements.
What We’re Looking For
  • Qualified Social Worker, registered with Social Work England.
  • Experience in adult social care, including Care Act assessments and safeguarding.
  • Knowledge of mental capacity assessments and best interest decision-making.
  • Strong communication and interpersonal skills, with the ability to work collaboratively.
  • A commitment to strength-based and person-centred practice.
