About the Role
An exciting opportunity has arisen for an experienced Social Worker to join the South Community Adult Social Care Team at Bristol City Council. This is a fast-paced and rewarding role where you will support adults in the community by completing assessments, reviews, and support planning under the Care Act 2014.
You will work in a multi-disciplinary environment, collaborating with health, housing, and voluntary organisations to deliver person-centred and strength-based support to Bristol’s diverse communities.
Key Responsibilities
- Undertake Care Act assessments, carer assessments, and reviews, ensuring individuals receive the right level of support.
- Develop support plans that promote independence, choice, and well-being.
- Complete mental capacity assessments and best interest decisions where required.
- Lead on safeguarding enquiries, working in line with safeguarding policies and procedures.
- Work closely with health, housing, and voluntary sector partners to ensure effective multi-agency support.
- Maintain accurate and timely records in line with statutory requirements.
- Qualified Social Worker, registered with Social Work England.
- Experience in adult social care, including Care Act assessments and safeguarding.
- Knowledge of mental capacity assessments and best interest decision-making.
- Strong communication and interpersonal skills, with the ability to work collaboratively.
