We’re seeking a Social Care Assessor to join our Hospital Discharge Team on an initial 3-month contract, with the possibility of extension. This part-time position is for 17.5 hours per week.
What You’ll Do:
- Assess and support patients’ social care needs as they prepare to leave the hospital.
- Work closely with healthcare professionals to ensure safe and effective discharge plans.
- Ensure compliance with national and departmental guidelines while delivering high-quality care.
- Provide vital support to individuals during a critical time in their recovery journey.
- Experience in social care assessments, ideally in a hospital or health-related environment.
- A compassionate and empathetic approach to supporting vulnerable individuals.
- Strong communication skills and the ability to collaborate effectively with multi-disciplinary teams.
- Someone who is proactive, adaptable, and dedicated to improving patient outcomes.
