Hospital Discharge Social Worker

Job Description

Responsibilities:
In this role, you will be responsible for:

  • Hospital Discharge: Coordinating and facilitating safe and timely discharges from hospitals, ensuring all necessary support is in place for a smooth transition back to the community or to appropriate care settings
  • Triage/Duty Services: Responding to urgent cases, assessing needs, and making informed decisions to provide the right level of support at the right time
  • D2A Reviews: Conducting Discharge to Assess reviews, assessing individuals in their own homes or care settings to determine ongoing care and support needs
  • CHC MDTs: Participating in Continuing Healthcare Multi-Disciplinary Team meetings to assess and review eligibility for CHC funding and to ensure appropriate care plans are in place
  • Hospital Avoidance: Working proactively to prevent unnecessary hospital admissions by providing timely interventions and support in the community

Requirements:

  • A degree in Social Work and registration with SSSC
  • Experience in adult social care, particularly in hospital discharge, triage/duty, or similar roles
  • Access to your own car
  • Enhanced PVG
