Service Care Solutions are currently recruiting for a Band 5 Community Psychiatric Nurse to join a Dementia team in Shrewsbury. This is a fantastic opportunity to work within a supportive and well-established NHS community team.
Key Responsibilities:
- Manage a defined caseload under the supervision of a Band 6 Nurse, including specialist assessments, care planning, and evaluation of treatment plans in line with Care Programme Approach (CPA) and Single Assessment Process (SAP).
- Contribute to the setting and monitoring of quality standards, in line with clinical guidance and Trust policies.
- Administer medication in accordance with NMC guidelines and Trust procedures.
- Assess individual care needs and deliver tailored interventions based on agreed care plans and pathway guidance, including both pharmacological and psychologically informed approaches.
- Complete and contribute to comprehensive risk assessments and risk management plans, promoting positive risk-taking to enhance quality of life and independence.
- Maintain accurate documentation and provide verbal and written reports on patient progress and conditions.
- Use agreed outcome measures to assess and review the effectiveness of interventions as part of the care planning process.
- Support training and education of students, colleagues, carers, and service users through both formal and informal means.
- Registered Mental Health Nurse (RMN) qualification
- Experience working with individuals with dementia in a community setting
- Valid NMC registration
- Enhanced DBS – Adults
