Key Responsibilities:
- Provide high-quality, patient-focused care in community settings.
- Conduct clinical assessments and deliver nursing interventions such as wound care, medication administration, and chronic disease management.
- Support patients and families with health education, promoting independence and well-being.
- Collaborate with multidisciplinary teams to ensure a seamless patient care experience.
- Maintain accurate, up-to-date patient records and documentation.
- Registered Nurse with current NMC registration
- Previous experience in community nursing is preferred, but not essential.
- Strong communication, assessment, and organizational skills.
- Ability to work independently and manage your own caseload.
- Flexible and reliable, with the ability to work various shifts as required.
